INDIVIDUAL MEDIA RELEASE

For Members of Challenge Success Teams

During conferences and events for School Program teams, we often take photos and videos of participants to document the important work of our schools. Challenge Success requests your permission to use these images in specially prepared materials to train educators, increase public awareness of our programs, and highlight our work through the use of educational and promotional videos, social media (e.g., Twitter, Facebook), newsletters, brochures, annual reports, our website, and other such media.

By consenting, you understand that photos, videos, and audio recordings may occasionally be identified with an individual or school name. You understand that all materials filmed, recorded, or produced using these images are considered the intellectual property of Challenge Success and no part may be reused without express consent of Challenge Success. In addition, you understand that this consent does not have an expiration date and that you may contact Challenge Success at any time to revoke your consent.

School Media Release

For Leaders of Challenge Success Teams

Challenge Success requests your permission to name your school in photos and videos taken during our annual conferences and other related events and to use photos from your school’s public website and any relevant media shared with us by your school team.  These materials may be used by Challenge Success to increase public awareness of our programs and to highlight our work through the use of educational and promotional videos, social media (e.g., Twitter, Facebook), newsletters, brochures, annual reports, our website, and other such media.  No individual names or contact information will be released without additional consent.  

By consenting, you understand that all materials recorded or produced using these images are considered the intellectual property of Challenge Success and no part may be reused without express consent of Challenge Success. 

Please note:  Declining to give your consent below will in no way affect your school’s eligibility to participate in the conference or other Challenge Success programs.
